The reason you are asked to complete a consultation form on your first visit to any 'reputable' spa - and I stress, reputable, (because if you are not asked to fill one in then you need to be asking what sort of facility it is!) is pretty much like going to the doctor for the first time. The therapist is working with your face, skin and body - we need to be aware of any existing conditions that could cause a problem if we were not to know about it. If you are taking any medications, have any allergies, and most importantly any medical history or injuries including surgeries, pregnancy etc. This is for your and the therapist safety, as most importantly we want you to be able to enjoy your treatment.
The consultation form is completely confidential and the details are logged on your personal file so that if you are to return on a regular basis we do not have to keep completing forms. Usually, a good spa will check on each visit just to make sure nothing has changed since your last visit, if it has, then your details will be updated.
The completed form will be kept securely in the Spa. Staff working in the Spa have all signed a confidentiality clause to ensure they do not divulge any client information.
